If the patient is 18 or older and wishes for a parent or guardian to become or remain a proxy account holder with joint access, the patient and proxy must complete an authorization and consent form

Specific questions regarding care and treatment of the patient will not be processed or responded to electronically. If you have a medical emergency or have a question regarding the care or treatment of a patient, please do not use the messaging feature provided through Patient Portal. Instead, contact your healthcare professional directly or call 911. This site is not a substitute for appropriate and timely contact with your physician.

VI. Confidentiality
CCHC is committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored in the Patient Portal as is given to medical information stored by CCHC in any other medium, and pursuant to and in accordance with the terms of CCHC’s Notice of Privacy Practices.
VII. Termination of Access
CCHC may restrict or terminate access to the Patient Portal at any time. If access is restricted for planned maintenance of the Patient Portal system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to the Patient Portal without advance notice. If your use of the Patient Portal violates these Terms and Conditions, CCHC reserves the right to permanently terminate your access to the Patient Portal.
